If you were impacted in any way by the recent winter storm in Louisiana, the Governor's Office of Homeland Security and Emergency Preparedness wants to know.

All Louisiana resident affected by the freezing weather conditions are urged to fill out a self-supporting survey called the Louisiana Disaster Preliminary Damage Self-Report so local and state officials can understand the scope of damage and how to proceed with recovery efforts. The survey is voluntary and can be completed online at damage.la.gov.

Please be advised that this survey is a statewide damage assessment to collect data and in no way should replace a damage claim to your insurance carrier. Furthermore, the survey result does not constitute there will be any assistance or federal disaster relief awarded.
